Patient care documentation is often taught along with general the communications section in EMT classes. In this introductory session we'll discuss the relationship that exists between our patient assessment and documentation and introduce the acronym LOCATE as a partner to SOAPE. The need to evaluate the patient environment as well as the patient is the focus. We'll briefly cover the other components of documentation as well.
